Transcranial HIFU therapy for brain tumor


The major obstacle for HIFU technology in brain tumor therapy is skull bone as the focused US field is disturbed and most of the US energy is absorbed by the skull bone. These difficulties have been overcome for the development of adaptive focusing techniques and large-sized phased array US transducers. Trans-cranial MRgFUS has been established and the clinical potential applications are researched in a fast-growing field of neuropathic pain and brain tumor therapy. MRgFUS system may be used to temporarily disrupt the blood-brain barrier for drug delivery to brain tumors.

A pilot study included 15 patients who underwent MRgFUS for the treatment of medication-refractory essential tremor. The results showed total tremor scores improved from 54.9 to 24.3 (P=0.001). The application of MRgFUS in 13 patients with chronic Parkinson's disease was reported. The authors showed the accuracy, safety and feasibility of the MRgFUS pallidothalamic tractotomy.

The clinical feasibility of trans-cranial MRgFUS was evaluated by McDannold et al.  Three glioblastoma patients underwent this procedure of multiple focused ultrasound exposures. The authors demonstrated the beam from HIFU at the brain with the magnetic resonance temperature imaging. However, MRgFUS seemed not to achieve thermal coagulation.

Trans-cranial MRgFUS could be a treatment option for brain diseases. HIFU modality for functional neurosurgery was shown by a clinical study where 9 patients suffering from chronic neuropathic pain were included and all treatments were all well tolerated. The results show the helpful clinical utility of trans-cranial HIFU modality for brain functional diseases. Large randomized clinical trials are needed to assess the safety and efficacy of trans-cranial HIFU therapy for brain tumor and neuropathic pain.

New technique allows ultrasound to pass through metal and bone


Ultrasound imaging is one of the most commonly used methods of medical diagnosis and treatment. It works by emitting high frequency audio waves; when these waves bounce off an object, they return to the ultrasound equipment. This equipment then translates the waves into an image. However, certain materials, such as bone or metal, have particular characteristics that block or distort ultrasound waves. The presence of these materials, known as aberrating layers, can decrease the accuracy and effectiveness of ultrasound imaging.

A new technique developed by researchers in the College solves this problem by using customized structures that compensate for the distortion usually caused by aberrating layers.

"In effect, it's as if the aberrating layer isn't even there," says Dr. Yun Jing, an assistant professor of mechanical and aerospace engineering.

In order to address the problem of aberrating layers, the researchers designed customized metamaterial structures that can account for and offset the acoustic properties of the aberrating layer. These metamaterial structures use a series of membranes and small tubes to achieve the desired acoustic characteristics.

This new technique has been tested by the researchers using computer simulations with impressive results. In these simulations, only about 28 percent of sound wave energy makes it past an aberrating layer of bone without the metamaterial structure in place. With the metamaterial structure, however, 88 percent of ultrasound wave energy passes through the aberrating layer.

This technique is a significant development for the use of ultrasound imaging in both medical and industrial settings.

"… (This) will make it easier for medical professionals to use ultrasound imaging for diagnostic or therapeutic applications, such as monitoring blood flow in the brain or to treat brain tumors. This has been difficult in the past because the skull distorts the ultrasound's acoustic field." - says Tarry Chen Shen, a Ph.D. student in the Department of Mechanical and Aerospace Engineering and lead author of a paper on the work.


According to Jing, the technique can also be used in industrial settings by allowing for the detection of cracks in airplane wings under the wing's outer layer of metal.

Θεραπεία με κρουστικό υπέρηχο (ESWT)


Τα κρουστικά κύματα έχουν φέρει επανάσταση στα πεδία της ουρολογίας και της ορθοπεδικής. Επιτρέπουν την πραγματοποίηση διαδικασιών μέσα στον οργανισμό δίχως να χυθεί ούτε μια σταγόνα αίμα. Μετά την πρώτη χρήση τους στη θεραπεία των ουρικών λίθων στην ουροδόχο κύστη στις αρχές του 1980, τα κρουστικά κύματα σύντομα κέρδισαν την παγκόσμια αναγνώριση. Η χρήση των κρουστικών κυμάτων έγινε γρήγορα μια αποτελεσματική μέθοδο θεραπείας στο πεδίο της ουρολογίας. Σήμερα, περίπου το 70% όλων των ασθενών που υπεβλήθηκαν σε θεραπεία νεφρού ή ουρικών λίθων θεραπεύτηκαν με εξωσωματικά κρουστικά κύματα (Λιθοτριψία). Παράλληλα, οι έρευνες στράφηκαν προς άλλες ενδείξεις στο πεδίο της ογκολογίας, της χειρουργικής, της γαστρεντερολογίας και της ορθοπεδικής. Λόγω των πολλά υποσχόμενων αποτελεσμάτων, οι μελέτες επικεντρώθηκαν περισσότερο στην ορθοπεδική.

Κατά τη διάρκεια ενός ερευνητικού προγράμματος πάνω στις παρενέργειες των ωστικών κυμάτων, μια Βουλγαρική/Γερμανική ερευνητική ομάδα ανακάλυψε την επουλωτική επίδραση των ωστικών κυμάτων στη θεραπεία των καταγμάτων. Παρατηρήθηκε επίσης ότι, μετά την εξωσωματική αγωγή με κρούστικά κύματα, πολλοί ασθενείς ανακουφίστηκαν από τον πόνο στην περιοχή, η οποία υπεβλήθη στην αγωγή. Αυτό το εύρημα παρότρυνε τη χρήση των ωστικών κυμάτων με σκοπό την αντιμετώπιση του πόνου, κυρίως του πόνου στους μαλακούς ιστούς κοντά στα οστά.

Ο κρουστικός υπέρηχος είναι ένα μέσο αποκατάστασης, που θεραπεύει με μεγάλη επιτυχία χρόνιες μυοσκελετικές παθήσεις, που δεν ανταποκρίνονται στις κλασικές μεθόδους αποκατάστασης. Ο κρουστικός υπέρηχος είναι μια μέθοδος θεραπείας, η οποία διοχετεύει κρουστικά κύματα σε συγκεκριμένους ιστούς του σώματος, με αποτέλεσμα την αύξηση της αιματικής ροής και του μεταβολισμού στην προσβεβλημένη περιοχή. O τρόπος δράσης των κρουστικών κυμάτων είναι η μετατροπή μιας χρόνιας φλεγμονώδους διαδικασίας όπως η τενοντίτιδα, σε οξεία όπου ο οργανισμός είναι ικανός να θεραπεύσει.

Η χειρουργική αίθουσα HIFU
Τα κρουστικά κύματα ενεργοποιούν τις διαδικασίες αυτοεπούλωσης του οργανισμού αποδιοργανώνοντας π.χ τα ασβεστώματα στους τένοντες, με αποτέλεσμα να διεγείρει την αναγεννητική ικανότητα των κυττάρων, που σαν τελικό όφελος έχει την αύξηση των μηχανικών ιδιοτήτων των ιστών και τη μείωση του πόνου. Τα κρουστικά κύματα έχουν ως πλεονεκτήματα να ανακουφίζουν άμεσα από τους πόνους και να μην εμφανίζουν ανεπιθύμητες ενέργειες (εφόσον έχουν σωστή εφαρμογή). Η θεραπεία έχει μικρή διάρκεια περίπου 10 λεπτά…
